Dr. Trieu Ho

Cardiac Electrophysiologist
Dr. Trieu Ho founded the Premier Heart Center with a mission of providing compassionate, personalized care, evidence-based, and comprehensive management approach in an independent and a boutique practice setting that is tailored to a patient’s conditions, expectations, and symptoms. He embraces the honor and the privilege of being on a patient’s health care provider team with the utmost care and sense of responsibility.
Dr. Ho is a board-certified by the American Board of Internal Medicine in the field of clinical cardiac electrophysiology from 2009 to present, with a passion for diagnosing and treating heart rhythm disorders. He earned his medical degree from Texas A&M HSC College of Medicine and completed his residency in Internal Medicine at UT Houston Medical School, followed by specialized fellowship training in general cardiology, clinical cardiac electrophysiology, and in advanced clinical cardiac electrophysiology at UT Houston Medical School. Dr. Ho has extensive expertise in managing complex arrhythmias, including atrial fibrillation, ventricular tachycardia, and other cardiac rhythm disturbances. Dr. Ho has co-authored several chapters in multiple books, participated in research and publication, as well as presented at national meeting conference.
As a dedicated specialist, Dr. Ho employs cutting-edge diagnostic tools and treatment techniques, such as catheter ablation, pacemaker implantation, and defibrillator management, to help patients achieve optimal heart health. He is committed to providing personalized care, taking the time to understand each patient’s unique condition and tailoring treatment plans to their individual needs.
Dr. Ho is an active member of the Heart Rhythm Society, and the American College of Cardiology, where he collaborates with peers to advance the field of electrophysiology. His approach to care is grounded in patient education and empowerment, ensuring that his patients are well-informed about their heart health and treatment options.
Outside of the clinic, Dr. Ho enjoys traveling and spending time with his family and is dedicated to maintaining a healthy and balanced lifestyle.

Cardiology Nurse Practitioner
Sarah Breitkreutz is an experienced cardiology nurse practitioner dedicated to providing comprehensive and compassionate care to patients with heart conditions. She earned her Master of Science in Nursing (MSN) from The University of Texas at Arlington and is board-certified as a family nurse practitioner. With a focus on cardiovascular health, Sarah works alongside cardiologists to diagnose, treat, and manage a wide range of heart-related conditions.
With a patient-first philosophy, Sarah is committed to building strong relationships with her patients, helping them understand their heart health and empowering them to make informed decisions about their care. Her clinical expertise includes conducting heart health assessments, managing chronic cardiac conditions, and promoting preventive strategies to improve overall well-being.
Sarah is staying updated on the latest advancements in cardiology and evidence-based practices. Her passion for patient education ensures that every patient receives the knowledge and support they need to take control of their heart health.

Sarah Montgomery

Cardiology Nurse Practitioner
Sarah Montgomery is a Nurse Practitioner specializing in cardiac electrophysiology, with a strong clinical background in emergency medicine and cardiology. She is dedicated to providing high-quality, patient-centered care to individuals with heart rhythm disorders and complex cardiovascular conditions.
Sarah earned her Master of Science in Nursing from the University of Texas at Arlington in 2014 and completed a post-graduate internship in emergency medicine. Her advanced training and clinical experience have strengthened her diagnostic skills and comprehensive approach to patient care.
In her current role, Sarah works collaboratively with the electrophysiology team to assess, diagnose, and manage patients with arrhythmias, including atrial fibrillation and other rhythm disorders. She places a strong emphasis on patient education, clear communication, and individualized treatment planning.
Outside of work, Sarah enjoys staying active through Pilates and weightlifting, and spending time outdoors playing disc golf. She believes that maintaining a healthy, balanced lifestyle supports both personal well-being and high-quality patient care.
